Please bring your photo ID, most recent insurance card(s), medications (or a list of current medications), and a method of payment if your insurance requires a co-pay, deductible or co-insurance.
Many appointments and almost all surgeries require a period of fasting beforehand. If you are unsure, please call our office at 903-533-8702.
We accept most commercial insurances along with Medicare and Medicaid. We will verify your benefits before your appointment and provide you with an estimated cost of your visit or surgery.
Depending on your insurance, we may be required to obtain a referral from your PCP. This most commonly applies to HMO policies. We will facilitate the referral process, if necessary.
We are proud to be an independent, physician owned practice and we currently have privileges at UT Health East Texas and Nacogdoches Memorial Hospital.
